 +Immunization of Students Protocol\\
 +School Board Approved: April 7, 2014**

 +Children must have proof of all state required immunizations,​ or valid exemptions, in order to be admitted to school. Documentation of immunity by confirming laboratory test results is acceptable.

 +A child may be enrolled under conditional enrollment when the parent or guardian provides:
 +
 +  * Documentation of at least one dose of each required vaccine.
 +  * The appointment date for the next due dose of required vaccine.
 +
 +A child shall be exempted from the above immunizations if he/she presents evidence from his/her physician that immunization will be detrimental to his/her health. A child may also be excused from immunization for religious reasons, upon the signing of a notarized form by the parent(s) or guardian(s) stating that the child has not been immunized because of religious beliefs.

 +Failure to comply with this provision will result in the child’s exclusion from school.
