Immunization of Students Protocol
School Board Approved: April 7, 2014

Children must have proof of all state required immunizations, or valid exemptions, in order to be admitted to school. Documentation of immunity by confirming laboratory test results is acceptable.

A child may be enrolled under conditional enrollment when the parent or guardian provides:

A child shall be exempted from the above immunizations if he/she presents evidence from his/her physician that immunization will be detrimental to his/her health. A child may also be excused from immunization for religious reasons, upon the signing of a notarized form by the parent(s) or guardian(s) stating that the child has not been immunized because of religious beliefs.

Failure to comply with this provision will result in the child’s exclusion from school.
